Application for summer 2009
This year we will again be using an online application process. When
you begin an application, you will use your email address as your user
name and you will select a password. You can go back to your
application later to fill in more pages or make changes. Please read
the online application
instructions before starting your application.
When you have completed the online process, you should print your
application to submit at interview night on October 14th (see printing
instructions below).

Access/make additions or changes to an application
In addition, certain forms requiring signatures still need to be
submitted in hard copy (see list below). These can be obtained from the
paper application, which will be provided at the orientation
sessions or can be downloaded below.
If you are not able to complete the online application by October
14th, you may complete pages 3-6 of the paper application to submit at
the interview night. If you are accepted into the Amigos program, you
will then need to re-enter this information online at a later
date.
Printing your online application

Enter your email address and password.
At the bottom of the next screen, "Click here for a printer friendly
summary of your application".
The forms required at interview night from the paper application as are follows:
- Consent and Release Agreement-- pages 11, 12, 13
- Medical Release -- page 14 (for volunteers who will be 18 or older prior to their return from the field next summer)
- Standards of Personal Conduct and Community Behavior--page 15
- Financial Assistance --pages 27 &28 (optional)
- Copy of first page of passport (if applicant already has a
passport)
The following forms also need to be submitted by interview night.
They must be placed in a sealed white envelope with the applicant's
name on front:
- Letter of Medical Disclosure -- page 17
- Confidential Health Form I -- pages 19, 20
