Application for summer 2012
Marin Amigos uses an online application process. When you save an application, you will use your email address as your user name and you will select a password. You can go back to your application later to fill in more pages or make changes.
Click on the link to start your new application or, click on this link to make additions or changes to your application.
In addition,as part of the application process, certain supplemental forms are required to be submitted and can be downloaded by clicking on the links below
The following form is due on Interview night--September 27, 2011:
If accepted to the Amigos program, the following forms requiring signatures need to be submitted in hard copy at our first meeting on October 11, 2011:

Chapter Participation and Financial Agreement
- Training Medical Release Form
- Consent and Release Form
- Standards of Conduct Form
- Letter of Health Disclosure
- Confidential Health Form I
- Application for Financial Assistance (required only if you are requesting financial aid)
- Confidential Health Form II (to be completed by a physician)
- IO Financial Assistance Form (optional-- if you wish to apply for an additional International Office scholarship)
